Eleventh Street Family Health Services of Drexel

Fitness Center Assistant

Eleventh Street Family Health Services of Drexel University is a nurse-managed Federally Qualified Health Center (FQHC) whose mission is to provide quality, comprehensive health services to all the people it serves with special attention to vulnerable people and residents of public housing communities, as well as provide an exemplary model of nurse managed, community based care for the education of health professions students and for faculty practice. The Center is located in Lower North Philadelphia and serves over 2,000 people that the federal government has designated as “medically underserved.”  Nurse practitioners and other staff provide on-site primary and preventative health services for all community residents, without regard to their ability to pay.  The Center strives to become a “Healthy Living Center,” by offering trans-disciplinary clinical services and health promotion programs in partnership with the local community.   
           
The member will become part of a dynamic team developing and implementing wellness programs.  They will work closely with the Physical Therapist and Holistic Health Counselor to monitor all activity in the fitness center.  The member will promote health and wellness within the surrounding community and will supervise and instruct clients in safe, effective, and age/condition-appropriate exercises and activities during open hours at the Fitness Center.

Health Education and Outreach Program Assistant

The member will maintain contact with parents and children in our many education programs with follow-up and reminder phone calls, and work with the Center staff to create, implement and evaluate programs and activities; assist and coordinate support groups, health workshops or activities of educational consultants, including nutrition, diabetes and smoking cessation programs; write, distribute and collaborate with the Center’s diverse staff on our monthly newsletter, which highlights all programs and services offered; and develop health education materials and handouts that are culturally and literacy appropriate for distribution within the health center and surrounding community.

Social Work Assistant

The member will assess patient eligibility for various health insurance programs and assist them in applying for these programs with an emphasis on insurance for children (CHIP) and the MA Family Planning Waiver,  Select Plan for Women; enroll patients in prescription assistance programs either while awaiting insurance or because they are not eligible for insurance; serve as an advocate with patients when they need to contact community agencies; assist patients in organizing appointments, arranging transportation or providing other case management services under the supervision of the Social Service Manager; and participate in the center’s other wellness and social service initiatives such Reach Out and Read Program, mammogram screenings, turkey and toy drives, health fairs, etc.

Abbotsford/Falls – Family Practice and Counseling

Network Chronic Care Service Worker

The Family Practice and Counseling Network (FPCN) is a program of Resources for Human Development, Inc. whose mission is to promote long-term health by providing quality, compassionate and affordable preventive, primary, dental and behavioral health care to all of its patients with special focus on residents of public housing. The public housing population served has significant socio-economic and cultural barriers to adequate health care and there is a lack of culturally appropriate services relevant to the majority of the population.  At our Abbottsford Falls site we provide primary care services, our state of the art facility provides numerous support and education programs focusing on youth, individuals and families living with cancer, diabetes, obesity, smoking cessation.  Our Outreach staff makes home visits to educate children and their parents about asthma and lead in the home and our social service department provides assistance to patients seeking insurance, referrals to other community agencies.   Additionally, FPCN provides behavioral health services and these behavioral health services are becoming more fully integrated with primary care.

Diversified Community Services

Community Health Educator

Diversified Community Services (DCS) is a multi-purpose social service agency situated in Point Breeze, a low-income, predominantly African-American neighborhood.  Our mission is to enable children and families to realize their full potential and achieve self-reliance. Nearly 8,000 children, youth, and families participate in a wide array of services that stress education and skill building, highlighted by the following: two nationally accredited early childhood education centers; after-school enrichment programs; comprehensive housing counseling & financial education to promote home ownership and community stability; parenting education to improve parenting skills and prevent abuse/neglect.

The member will design and implement an age-appropriate healthy life-styles curriculum in consultation with supervisors and other health experts; provide the hands-on instruction in DCS classroom settings; organize periodic healthy eating/lifestyle education forums for parents in conjunction with other DCS staff/supervisors; and assist appropriate DCS staff and other community representatives in planning and securing a farmer’s market in Point Breeze.

Health Annex as site of the Family Practice and Counseling Network

Social Work Assistant

The Health Annex is a nurse-managed Federally Qualified Health Center (FQHC) whose mission is to provide quality, comprehensive health services to all the people it serves with special attention to vulnerable people and residents of public housing communities.  The Center is located in Southwest Philadelphia and serves a predominantly African-American population that the federal government has designated as “medically underserved.”  Nurse practitioners and other staff provide on-site primary and preventative health services for all community residents, without regard to their ability to pay.  In addition to primary and preventive services, the health center provides diagnostic testing and screening; HIV-testing and counseling; family planning, pre-natal, obstetric; hearing and vision testing; nutrition and diabetes education; social work; behavioral health services; asthma safe kids and lead safe babies programs and dental care.  As an every-changing health center, we have an active social service department   helping patients with insurance, benefits counseling, providing advocacy and referral, and arranging transportation and are always seeking new ways to assist our patients.  

The member will enroll patients in prescription assistance programs; serve as an advocate with patients when they need to contact community agencies; assist with implementation and outreach for our new HIV rapid testing program; review and participate in the site’s new chronic care initiative including running data reports and assisting as needed with overall implementation of the program; and assist patients in organizing appointments, arranging transportation or providing other case management services under the supervision of the Social Service Manager.

Health Federation of Philadelphia – Early Head Start

Family Health and Wellness Advocate

The purpose of the EHS program is to support parents in their individual growth and development while they meet and support the developmental needs of their child(ren) to reach their full potential.  A full range of multisystems family centered, comprehensive and coordinated EHS services, including high quality health, education, family involvement, nutrition, and social services is provided to low-income pregnant teenagers/young adults (13-25 years old) and to teenage/young adult parents of infants and toddlers (0-3 years old) living in the North Central Empowerment Zone (NCEZ) in Philadelphia.  A total of 75 infants and toddlers or pregnant teens/young adults and their families will be served in the program.

The continuum of services is delivered to children and their families using a home based option with a community site in North Philadelphia.  Services include home visits, parent-child groups, educational and support groups, individual and family counseling, case management, and referrals to community agencies and are provided by a multidisciplinary team of child/ family advocates, social workers, family and community involvement specialists, child development specialist, registered nurse and registered nutritionist.

The member will work with team members to link families with appropriate community resources; participates and assist in community outreach activities, recruitment, health fairs, special events and family activities; participate in ongoing in-service/staff development training programs; and provide health education to families.

Maria de los Santos Health Center (Delaware Valley Community Health)

Associate Health Educator

Delaware Valley Community Health is a Federally Qualified Community Health Center, Maria de los Santos Health Center (MdlS) is one of tee DVCH centers, and serves the Latino community of N. Philadelphia. The mission of DVCH is to provide quality, culturally competent primary care services, regardless of the patient’s ability to pay. We provide the full range of health services: adult medicine, pediatrics, ob/gyn, dentistry, mental health, social work and health education.

Maternity Care Coalition

Cribs for Kids Program Assistant

Maternity Care Coalition is a private nonprofit organization founded in 1980 by professionals and lay people concerned with the appalling rate of infant mortality and morbidity in Philadelphia. MCC’s mission is to improve maternal and child health and well being through the collaborative efforts of individuals, families, providers and communities. The agency achieves its mission through outreach in high-risk neighborhoods and advocacy at the local, state, and national levels.

The member will respond to requests for cribs from the community and provide assessments and education; conduct home visits with families to deliver and set-up cribs and provide safe sleep/SIDS risk reduction education; lead 2 to 4 monthly community education workshops on safe sleep at sites throughout Philadelphia, including teen parenting programs at the Philadelphia high schools; attend community meetings, health fairs, and other events; assist in the training of all other health social service providers about SIDS risk reduction and safe sleep environment; and learn about and make contact with community resources needed by clients.

National Nursing Centers Consortium

Health Outreach Educator

National Nursing Centers Consortium operates nurse-managed health centers dedicated to eliminating health disparities in underserved communities. They use a model of care emphasizing health promotion through education, focusing on preventative measures.

The member will facilitate exercise programs as part of the ‘To Our Children’s Future With Health Inc’, at residential treatment centers, and the ‘Go Red for Women’ activities, a partnership between the NNCC, the State of Pa and the American Heart Association; work with predominantly African American women, children, and adolescents who are at increased risk for cardiovascular diseases by conducting classes on line dancing, light aerobic exercises, walking classes, etc.; facilitate nutrition education classes, with a focus on healthy cooking and eating habits; educate students about risks, symptoms, and prevention of various diseases and conditions related to cardiovascular health.

Penrose Elementary School

Penrose School Health and Fitness Educator

The Penrose School is dedicated to improving the academic and health status of our children through health and fitness programs and involvement of families in the health care of their children. The school has fitness programs after-school five days a week and also in school programs to improve the health of our children. Penrose school is dedicated to academic achievement and good health for the school and community.

The members will assist the school nurse and physical education teachers in implementing a Coordinated School Health and Fitness program; implement an after-school fitness program for one hundred children; assist the school nurse in health screenings; work with families to obtain health insurance and medical care; assist the school nurse and counselor in obtaining mental health assistance for students; plan a summer health program for students in a local housing complex; run a socialized recess program; and coordinate a fitness program for special needs kids.

Philadelphia Department of Public Health - Ambulatory Health Services

Patient Assistance Program Advocate

The Philadelphia Department of Public Health Ambulatory Health Services Division has a total of 8 health care centers which provide comprehensive primary medical care and preventative services to underserved communities. Services offered at the health centers include family medicine (adult and pediatric), dental, family planning, and pre-natal care.

Members will assist patients with enrollment in Pharmaceutical Medication Assistance Programs (PMAPs).  Members will access applications from pharmaceutical companies and RxAssist software; work with patients and staff to complete applications and facilitate the process collecting necessary documentation (from patients and providers); track the status of applications through phone calls to companies and patients; follow-up through phone calls and direct mail with patients whose medications have arrived; ensure correct medications are distributed to patients and tracked appropriately; provide support and education to health center patients concerning PAP application process.
